We will give every effort to make sure your first visit is comfortable, safe, and welcoming. Save time at your first visit by printing and filling out the forms below.

At your first visit:

• You will be asked to provide a medical history for the doctor to review.

• The doctor will examine your feet and may take an x-ray.

• The doctor will teach you about the diagnosis and help guide you in choosing the best treatment.

We offer the option to have virtual telehealth appointments for follow-up visits.

We accept Medicare, Medicaid, CHI/Uninet, Blue Cross, United Healthcare, Medica, Aetna, workers’ comp, and other types of insurance. For more information contact your insurance provider or contact our office.

We provide Spanish interpreters upon request.
